Your insurance coverage/payment is your responsibility. Please bring your insurance card with you every visit. We will also need a picture ID on your first visit. We collect your copay at time of service. For billing questions contact our billing office. You will receive a bill from the office after your insurance pays their portion. You will be responsible for deductibles as outlined by your insurance company. You may be required to pay all or a portion of your deductible at time of service. Bills can be paid online or by phone with a credit card. If you do not have insurance we require payment at time of service. We offer patients that pay cash at the time of service a discount. In the event of financial hardship, contact the billing office to discuss a payment plan. We are glad to help out when times are tough for you and your family.
Beginning January of 2017 we will charge a fee for no-show appointments. There will be a $50 fee for missed acute appointments and a $75 charge for missed physical appointments. We pay a service to remind all our patients of their pending appointment 2 days in advance. You receive a phone call with the date and time as a reminder. If you cannot make your appointment, please call the office at least 2 hours prior so that we may fill the slot with another patient. If you have 2 no-shows or multiple cancellations, you may be asked to leave the practice.
